Background: Cardiovascular disease in Germany is steadily on the increase. It is one of the main presentations in GP consultations and one of the leading causes of death. Long-term lifestyle change is necessary to successfully deal with the disease. However, only a minority of those affected are able to successfully implement such changes in their everyday lives. A major problem is the lack of time resources in practices, meaning that regular and patient-orientated risk-advice from general practitioners cannot be guaranteed.

For this reason, DECADE was developed as an intervention for GPs and their patients, and has already been successful in a recent pilot scheme, as published in the Deutschen Ärzteblatt.
