of the University Medical CenterYour disease and its treatment at the Medical Center – University of Freiburg can cause changes in your life situation. This may give rise to questions that we would be happy to inform and advise you and your relatives about. Our goal is to work with you to develop individual assistance for your personal situation. Our social counseling complements the medical and nursing care provided by the medical center – also with regard to discharge from the hospital. Please feel free to call us, or ask your doctor or the nursing team about our social services.

The University Medical Center Social Services include the following:
- Psychosocial counseling: Support on personal and social problems arising from your stay at the hospital
- Clarification of social legal claims
- Consultation on medical rehabilitation and organization of follow-up treatment
- Information on rights of the disabled and on occupational reintegration
- Information on healthcare proxy and on guardianship law as well as suggestions concerning legal guardianship
- Clarification of further care in the home environment
- Organization of family assistance for children in need of care
- Support in the search for short-term care, nursing homes, and hospice beds
- Contact on counseling services and self-help groups
