After long stays at the hospital, critically ill tumor patients generally wish to be able to return home.
Home care helps you and your loved ones to take this step.

We are qualified nurses and have additional training in oncology, specialized palliative care, and pain therapy.

We are part of the Palliative Care Team SPES and are the specialized outpatient palliative care (SAPV) provider at the Medical Center – University of Freiburg, assigned for organizational purposes to the Department of Palliative Care (in German). In addition to the home care team, a coordinator and a senior physician are also on duty for our patients.

We help you to plan the transition from hospital to home – if necessary and if you so desire, until the end of your life.

• We plan and organize the discharge of patients from the hospital early on and prepare the best possible nursing and medical care at their homes.
• We make it possible for tumor patients who would otherwise not be able to leave the hospital to live at home.
• We provide a personal contact person for the entire care period.
• We cooperate with hospital clinicians, family doctors, and specialists, as well as with nursing services and other healthcare institutions.
• We provide advice and guidance to patients, relatives, and nurses.
• We regularly monitor symptoms.
• We provide support and consultation on pain therapy
• We conduct special treatment procedures, such as port care, analgesia infusion pumps, etc.
• We do our utmost to avoid unplanned readmissions to the hospital.
• We accompany patients in the last phase of life and up to the end of life.
• We can be reached around the clock in the event of an emergency
